Return Merchandise Authorization Sheet

Use the below submission form to complete your RMA return sheet.
IMPORTANT: An authorized RMA number MUST be provided by a Skyline USA representative to successfully
complete the process.

Please carefully follow the directions. Failure to comply with instructions can/will result in rejection of returned merchandise

Customer must claim any damaged merchandise within 30 days of receipt of original merchandise

1. Customer must print and return RMA form, in proper completion, within 15 days of receipt of RMA number

2. RMA form must be returned along with the defective merchandise

3. RMA number must be clearly visible on the outside of all boxes

4. RMA number must be clearly visible on the outside of all boxes

Returned merchandise must be factory defected. Any products returned damaged by the customer will not be accepted.
Skyline USA can, at company discretion, void any claims if they do not fall within the parameters of the return policy.
Returned merchandise may only be applied toward exchange of same, or similar, product. Exchange must be claimed
within 60 days or the RMA will be void.
